School Enrollment Form Instructions

Instructions for Parents/Guardians

1. Date:

   • Write the date when you begin filling out this form.

2. League Information:

   • League Name: Write one of the following:
     • Spotsylvania County LL
     • SCLL
     • Spotsylvania County Little League
   • League ID#: Enter 208167.

3. Player/Student Name:

   • Print your child’s name exactly as it appears on their birth certificate to ensure consistency.

4. Division and Level Selection:

   • What to Select:
     • Choose the division and level your child is currently enrolled in.
     • If your child is pending a waiver, select the division they were originally enrolled in.
   • Special Instructions for Specific Divisions:
     • Coach Pitch & Machine Pitch Levels: Select Minors.
     • Challenger Division: Select Baseball and choose either:
       • Majors Level
       • Senior Level
       (based on your athlete's current division).

5. Parent/Guardian Address:

   • Provide your complete address, including Street, City/State, and Zip Code.

6. Parent/Guardian Signature:

   • A Parent/Guardian signature is mandatory. Forms without a signature cannot be processed.

Additional Reminders for Parents/Guardians

• Double-Check: Verify all details are accurate and legible.
• Required Once: This form is only needed once during the participant’s Little League career unless the school changes. If the school changes, a II(d) waiver must be completed.

Instructions for School Administrators (Principal, Vice Principal, or Authorized Registrar)

1. School Year:

   • Enter 2025-2026 as the academic year.

2. Enrollment Date:

   • Enter the date when the student was enrolled in school.
   • Important: This date must be October 1, 2025, or earlier.

3. Authorized Titles:

   • Use one of the following recognized titles in the Title field:
     • Principal
     • Vice Principal
     • Administrator
     • Registrar
   • Note: If the Secretary also serves as the Registrar, write Registrar in the title field.

4. Signature and Contact Information:

   • Sign and date the form.
   • Include:
     • Your printed name.
     • The school name.
     • The school’s complete address.
     • The school phone number.

Submitting the Form

• Once the form is completed and signed by both the Parent/Guardian and the School Administrator, it must be submitted to the Spotsylvania County Little League Player Agent.

  2. Hand Delivery: Bring the completed form to a league assessment event.
